How can I make an intro that fades out?

I wanna know how to make a intro that has something like. (Group) Presents, and then the (Group) Presents fades out and then it says. A game created by (______) Special thanks to ____ _____ and then fades out to a screen that has the game title about in the middle and at the bottom a button that says Play and also the background being part of the game rather then being black like before when it was doing the (group) presents and stuff.

If someone could help it would be very useful because im trying to create a game at the moment and I can't find out how to do it.
