How can you make smooth roads incline and turn at the same time?

I'm looking to create roads that are smooth turning left and right, but also inclining/declining all at the same time.

It's easy to make it smooth on one of the axes with tools like Archimedes, but once you try to make them both incline and turn is when you run into weird bumps in the road.

I'd imagine it has something to do with bezier curves and I know wedges are used, but I have no idea how to do the math behind it, and the current bezier road plugins don't work for inclines and turning at the same time, and the bezier path plugins are a little inaccurate and impossible to cleanly link multiple paths together.
