How would you make a player teleport to a different player's game using the players id?

I am wanting to create a teleportation system that would teleport a player, say "asd" to player "asd2" using asd2's id. Like if asd is in game 1, and asd2 is in game 2, I would like to teleport asd to asd2's server using his/her player ID.
