How Do I Make An RP Name GUI With Multiple Text Options?

I don't know if this counts as a request but here goes. I've been having trouble recently making an RP name GUI for my conquest roleplay game. Instead of having 1 TextBox to fill in, you have 4 for multiple important in-game characteristics(E.g Economy) I tried using a script from another RP name GUI but I dont know how to use the multiple inputs in the same RP Name.
