Click a player to open GUI that shows a description they input?

I currently have a a GUI that allows you to input a RP name and separate description that overrides the players nameplate. I would like to move the description to a separate GUI, so when you click on a player in game it opens a GUI to view the description they set.

Not looking for a script, just some guidance on how to begin setting it up would be helpful!
