I'm new to programming in Roblox. I could not find anything about doing permanent items, so I will ask here.
I have a script that adds the tool when you join the server, but when you die you lose it.
Is there a way that I can check if a specific user dies?
You can link the Died event of each player's Humanoid and pass that player every time that event fires. The Humanoid's Parent is the player's Character, which can be associated with the player itself using GetPlayerFromCharacter(), a function of the Players service. I'll leave you off with this, because we cannot supply code to you if you do not supply code for us to fix.

You can just clone add the tool to Player Backpack and StarterGear :D
