Why aren't hats being positioned correctly?

Hello!

I'm using :GetHumanoidDescriptionFromUserId() to create an NPC that automatically updates, however the issue with this is that a lot of accessories aren't positioned correctly for some reason.

I've tried re-applying the accessory to the character, however that doesn't seem to work, nor does anything else.
